Semaglutide and Tirzepatide

Semaglutide is a GLP-1 receptor agonist. It is a peptide comprised of 31 amino acids. It attaches to the GLP-1 receptor in the body to illicit its effects. GLP-1 receptors when activated can initiate insulin secretion, cause delayed gastric emptying and reduced appetite.

Tirzepatide is a dual GIP and GLP-1 receptor agonist. It uses this action to enhance insulin secretion. This improved glycemic control can result in reduction of body weight by 20% or more.

Both are a peptide therapy weight loss drug administered by injection. The treatments  are effective in helping patients who struggle to lose weight with diet and exercise alone achieve their desired results.

BEFORE USING THIS MEDICATION LET YOUR PROVIDER KNOW -

GIVING A SUBCUTANEOUS INJECTION

PREPARING TO GIVE MEDICATION BY SYRINGE

  1. Wash your hands and surfaces thoroughly.

  2. Assemble your supplies:

    • Medication

    • Syringe

    • Medication Log

    • Container for syringe and needle disposal

    • Sterile 2x2-inch gauze pad

    • Alcohol pads

LOCATING INJECTION SITES

Subcutaneous injections can be given in the arms, legs, or abdomen. Your providers will help you select the best sites to administer your medication at your consultations and check-ins.

  • Injection sites on the arms are just below the shoulder and just above the elbow.

  • Injection sites on the thighs are on the outer areas, below the groin and above the knee.

  • Injection sites on the abdomen are below ribs and above hip bones, avoiding the belly button.

Avoid areas where skin is burned, scarred, hardened, inflamed, swollen, or damaged by a previous injection. Also avoid scars and moles when injecting.

ROTATING INJECTION SITES

It is extremely important to rotate injection sites to keep skin healthy. Repeated injections in the same spot can cause scarring and hardening of fatty tissue that will interfere with uptake of medication. Each injection should be about 1 inch apart.

SAFE NEEDLE DISPOSAL

  • Place syringe in a hard plastic sharps container with a tightly secured lid.

  • Do not re-cap needles after use.

  • Keep the container out of the reach of children or pets.

  • Check with your local doctors office or hospital for disposal locations for used needles and syringes. Any of our patients can bring their sharps to our office for disposal. We do not provide sharps containers for at home use.

DRAWING UP MEDICATION

  1. Check the label for correct medication.

  2. Remove the soft metal or plastic cap protecting the top of the vial.

  3. Clean the exposed top of the vial using an alcohol swab.

  4. Remove syringe from the plastic or paper cover. If necessary, attach the needle securely. Avoid touching the needle.

  5. With the needle capped, pull back the plunger, filling the syringe with air equal to the amount of medication to be administered.

  6. Remove the cap covering the needle, and set it on its side to keep it clean.

  7. With the vial in an upright position, push down through the cleansed center of the vial, being careful not to bend the needle.

  8. Inject the air in the syringe into the vial.

    • If too little or no air is injected, withdrawing medication may be difficult.

    • If too much air is injected, the plunger may be forced out of the barrel causing medication to spill.

  9. Turn the vial upside down, with the needle remaining in the vial, making sure that the tip of the needle is completely covered by the medication.

  10. Pull back on the plunger to fill the syringe with the correct dose of medication. You may go slightly over the amount needed. If you see a lot of air in the syringe, you may need to push medication back into the vial and withdraw again slowly, making sure the needle tip stays in the solution.

  11. Once you have drawn the proper amount, take the needle out of the vial.

  12. Point the needle and syringe straight upwards and tap the syringe to move any air bubbles to the top.

  13. Once bubbles are at the top of syringe, gently push on the plunger to force the air out of syringe.

  14. Check to make sure you have the correct amount of medication in the syringe.

INJECTING MEDICATION

  1. Cleanse predetermined injection site with a back and forth motion using an alcohol pad.

  2. Quickly insert the needle all the way into skin at a 90 degree angle. Do not press down on the top of the plunger while piercing the skin.

  3. After needle is completely inserted into the skin, press down on the plunger to release medication into the subcutaneous layer at a slow, steady pace.

  4. Pull needle out of skin, and gently press a 2x2-inch gauze onto the insertion site. Apply band-aid if desired.

  5. Discard needle in sharps container and safely store out of reach.

  6. Store remaining medication in the refrigerator.
